Community Support Services
Community support services offer assistance with acquisition, retention, or improvement in self-help, socialization, and adaptive skills. Services include activities designed to keep participants engaged in their environment, develop and maintain relationships, and build on previously learned skills. Services shall be furnished consistent with the participant’s IPC, and include full access to the community to the same degree as community members who do not receive Medicaid home and community based services. Adult educational supports are an approved activity of this service.
Community support services shall not be scheduled or provided in the participant’s private residence. Services shall be furnished in a variety of settings in the community, and shall not be limited to only fixed- site or congregate settings. Activities and environments shall foster the acquisition of skills, appropriate behavior, greater independence, community networking, and personal choice. Transportation is a component of community support services and is included in the rate to providers.
Community support services shall focus on enabling the participant to attain or maintain his or her maximum functional level, and shall serve to reinforce skills or lessons taught in other settings, including skills learned during therapy services. Community support is a habilitation service. Training on objectives, which will be focused on community access, is expected as part of the provision of services, and progress will be documented and made available to the participant, legally authorized representative, and case manager each month.

Community support services are available for clients ages 18 and older.
The high level of care tiered rate for community support services will be available to participants who want to help building meaningful relationships and social connections in the community with a more individualized approach from the provider.
A relative provider (defined as a biological, adoptive, or step-parent) may provide this service.
Support with personal care needs, including medication assistance, is a component of the service, but shall not comprise more than 20% of the service. Personal care services won’t be able to be billed at the same time as this service.
Services will not exceed an average weekly amount of 35 hours if the individual receives a level 3 – 6 community living services.

Caregiver may:

Help problem-solve the person’s concerns related to daily living

Provide verbal, visual and/or touch guidance to help the person complete a task

Develop and demonstrate cues

Provide reminders and cueing systems to complete ADLs

Cue and/or provide intermittent physical assistance with dressing, grooming, eating, toileting, mobility, transferring and positioning

Cue and/or provide continual supervision and physical assistance with bathing, as needed

Help with cleaning, meal planning/preparation and shopping for household and personal needs

Help with budgeting, money management, and communication

Provide reminders about and assistance with exercises and other health maintenance or improvement activities

Help the person access activities, services, and resources that facilitate meaningful community integration and participation
Caregiver will not:

Specialized or adapted equipment for remote support

Transportation mileage

A person cannot receive Community Living Services if they receive any of the following:

Adult foster care
-
Consumer-directed community supports

Customized living
